Insurance, Payments, Forms and Medicare

I am Out Of Network for all insurance plans except Regular or Original Medicare.
Accepting assignment for Original Medicare means that if you have Medicare, Medicare will pay 80% of my charges at the Medicare rate for Psychotherapy Services when you are seen in my office as long as you have a mental health diagnosis and they determine there is a medical necessity for the visits. The balance (20%) is your co-pay. This fee may be covered by your Medicare Supplemental Insurance. My billing service submits all forms to Medicare for you.
Medicare Advantage Plans may be covered if you have Out of Network Benefits from a PPO plan. I am NOT in network for any HMO plans such as Kaiser, or Medicare Complete from United Healthcare.
Medicare insurance plans can be complicated to understand, so please ask me about them. Medicare Advantage plans have very limited networks.
Out of Network
I do work with clients who choose to see me when I am not in their insurance plans. Examples of this would be for Retirement Coaching, telephone, video therapy, and Caregiver Support/Education. Medicare does not pay for Tele-Mental Health Service at this time from my office. Clients who use their out of network benefits pay for sessions up front. However, I will have my Biller submit your sessions to the insurance company at your request.
Fees for Mental Health are $250.00 for the initial Intake visit. Psychotherapy is available at $200 for a 55-minute session.
Reports and Letters, or phone sessions are billed at prorated Rates depending upon the time involved.
I do not provide court testimony. If it becomes necessary, I charge a daily rate for my services. Please ask for the current rate.
Services for Retirement Coaching are NOT covered by Medicare or insurance. Medicare covers mental health services only.
Retirement Coaching is available in person, over the telephone or through Video.
These services can be provided National and Internationally.
Fees depend upon the services desired.
Caregiver support may be covered by insurance on a case by case basis if you are seen in my office. If you do use insurance, you would need to have a medical diagnosis for payment.
